This form is strictly a renewal form for PAGE members currently on payroll deduction transferring to a different school system. To update your information, please complete the Payroll Deduction Transfer Form and return the form to the PAGE office by fax to 770-216-8589 or mail to PAGE, P.O. Box 942270, Atlanta GA 31141.
