Payroll Deduction

With payroll deduction, the full amount of PAGE dues is deducted from your payroll check in the number of installments set by the central office of your system. Payroll deduction is automatically renewed each year unless PAGE is notified, in writing, 30 days prior to cancellation date.

Note: Please see list of payroll deduction deadline dates before submitting an application. (Applications submitted by Bank Draft, Checks and Credit Cards are accepted throughout the year.)

To apply for payroll deduction, please complete our Membership Application indicating that you would like payroll deduction and return the application to the address listed at the top of the form.
